Health safety protocols will be communicated before our events. AlphaWe adheres strictly to all National, State, County, and Event restrictions. Any participant who violates these rules could be ejected from the participation with no refunds.
PAYMENT OF FEES AND MEDIA RELEASE
The Participant or guardian signing the registration packet will be responsible for the payment of the selected costume as laid out in this registration form and will be considered in full understanding of the terms and conditions. Signee authorizes AlphaWe Art and Entertainment or its agents, consignees, or accredited personnel to photograph, film, broadcast, record, and videotape, them during the band’s performance (including all individual portrayals) and to incorporate the performance in a live or delayed broadcast, videotape, Compact Disc, DVD or any such audiovisual fixation. Participant gives ALPHAWE ART AND ENTERTAINMENT permission to use photos, video footage, and voice recorded at AlphaWe-related functions to be used for all promotional purposes (website, myspace, Facebook, magazine, flyers, posters, television, radio, Instagram, etc.) AlphaWe Art and Entertainment shall have the right to manufacture, sell distribute, and otherwise utilize throughout the world via direct delivery, electronic media, Internet, or otherwise representing the said performance.
PARTICIPANT LIABILITY WAVIER AND RELEASE
PARTICIPANT RELEASES AND HOLDS HARMLESS, ALPHAWE ART, CULTURE AND ENTERTAINMENT AND THEIR AGENTS AND/OR REPRESENTATIVES, FROM ANY LIABILITY FOR PERSONAL INJURY ARISING OUT OF MY PARTICIPATION IN TAMPA CARNIVAL OR RELATED EVENTS. PARTICIPANT ACKNOWLEDGES THAT THEY, BAND MEMBERS, VOLUNTEERS, OR THEIR GUESTS WILL NOT BE COVERED UNDER ANY OF THE SPONSOR LIABILITY INSURANCE FOR ANY INJURY INCURRED OR CAUSE AS A RESULT OF THEIR PARTICIPATION IN THE ABOVE-MENTIONED EVENTS/ACTIVITY BUT WOULD BE RESPONSIBLE THROUGH PERSONAL INSURANCE FOR ANY AND ALL MEDICAL EXPENSES INCURRED AS A RESULT OF INJURIES FROM THIS PARTICIPATION. PARTICIPANT UNDERSTANDS THEY ARE COMPLETELY RESPONSIBLE FOR ALL LIABILITIES, DAMAGES, AND INJURIES THEIR EMPLOYEES, THEIR VOLUNTEERS, OR THE PARTICIPANTS THEMSELF MAY CAUSE TO ALPHAWE ART, CULTURE, AND ENTERTAINMENT. & ITS ASSOCIATES, ITS MEMBERS, EMPLOYEES, AND ALL THIRD PARTIES (E.G.: SPECTATORS, BUYERS, ETC.) AS PART OF THEIR PARTICIPATION IN THIS EVENT INCLUDING DAMAGES OR INJURIES CAUSED BY THEIR EQUIPMENT, FOOD, DISPLAYS, VEHICLES, AND SUPPLIES AND INCLUDING ANY AND ALL INJURIES OR ILLNESSES THEIR PRODUCTS MAY CAUSE. PARTICIPANT HEREBY EXPRESSLY WAIVES AND RELEASES ALPHAWE ART, CULTURE AND ENTERTAINMENT & ITS ASSOCIATES FROM ANY AND ALL RIGHTS OR CLAIMS OF ANY NATURE WHATSOEVER THEY MAY HAVE AGAINST ALPHAWE ART, CULTURE AND ENTERTAINMENT & ITS ASSOCIATES, ITS MEMBERS, EMPLOYEES ARISING OUT OF, IN CONNECTION WITH, OR RESULTING FROM THE ABOVE EVENTS/ACTIVITY. IN ADDITION, BY REGISTERING AS A PARTICIPANT, THE SIGNEE IS AGREEING TO ALL TERMS AND CONDITIONS SET FORTH WITHIN.
